Waiver of Liability & Guidelines for DINOSAURS ROCK Dig Tour
Summer 2009
Aug. 23 - 25 or Aug. 28 - 30 

This form must be signed by each adult participant and dated and returned with your registration fee.  I, the undersigned accept and agree to the following: 

1. DINOSAURS ROCK. does not provide any type of medical or liability insurance to cover any possible injuries and accidents while I (and my family members) participate on this dig. I agree to be responsible for carrying my own medical and liability insurance for myself and any family members who attend the dig. 

2. I will utilize my own vehicle for transportation to the dig sites and I agree to provide my own liability, medical and auto insurance. I understand that DINOSAURS ROCK and its members do not provide any insurance for my vehicle and in the event of an accident or trouble, I agree that I assume all responsibility and will not hold DINOSAURS ROCK, its members or the landowner liable for any injuries that I (or family members) may incur as a result of travel to the dig site or any damage to my vehicle. 

3. I will not hold DINOSAURS ROCK., its personnel or the landowner liable for any damage to personal property (cameras, clothing, etc.) while on the dig site. 

4. Since this is an outdoor operation, the following situations must be taken into account. I understand field conditions may be hazardous at times and the following may occur:
·         Daytime temperatures can range from 70’s to 90’s in summertime
·         Relatively little shade is available and participants must be adequately prepared for long periods in the sun.
·         The terrain can be rugged with steep slopes, ravines, cliffs.
·         Local wildlife may be encountered at the dig site or when hiking looking for new sites.

5. I understand that public lands are nearby and in adherence with federal law, no collecting will be allowed on these lands while I am a participant of the DINOSAURS ROCK dig.

6. I will notify DINSOAURS ROCK personnel of any pre-existing medical conditions that could be exacerbated by the rigors of outdoor work. These include: asthma, heart condition, allergies to insect bites, or other pre-existing conditions. I will also notify DINOSAURS ROCK personnel prior to my arrival of any food allergies or special dietary needs, and if these exist, agree to provide my own special food supplies (DINOSAURS ROCK is supplying lunches) for my family as needed. 

7. If I bring children onto the dig sites it will be my responsibility to supervise them and make sure they behave in a safe and proper manner. Each child will be accompanied by a parent/guardian.   If I am traveling with a child on the DINOSAURS ROCK dig, I have the sole responsibility for ensuring that the child with whom I am traveling follows all rules of safety.  I release DINOSAURS ROCK and its members of all liability for any risks and/or injuries to the child with whom I am traveling.

8. I understand that I will not disclose the location of the dig site or publish any articles concerning this dig without the express permission and approval of DINOSAURS ROCK personnel. DINOSAURS ROCK reserves the right to review any papers prior to publication. 

9. I understand the fees do not include equipment for excavation at the dig site.  Participants agree to provide their own safety goggles and hammers.  Participants are forbidden to bring power tools, firearms or explosives onto DINOSAURS ROCK dig sites. 

10. I understand that collecting of materials at the dig sites will be at the discretion of DINOSAURS ROCK personnel. 

11. Cancellation Policy – I will incur cancellation penalty fees per person as follows:
                  60 days prior (up to 6/16/08) - $100 per person
                  59-8 days prior (up to 8/9/08) - $200 per person
                  7-1 days prior (8/16/08) - $300 per person
In the event DINOSAURS ROCK has to cancel the Dig Tour, due to any event or act outside and beyond our control, DINOSAURS ROCK liability is limited to a refund of the tour fee.

I, the undersigned understand the conditions outlined and by my signature below, I accept full responsibility for any injuries or hazards encountered for myself and any family members or relatives attending. I agree that I, any family members, or relatives attending will not hold DINOSAURS ROCK. or its personnel (inclusive of relatives or family members of DINOSAURS ROCK) or the landowner responsible or liable for any injuries that I, my family, relatives or friends may incur as a result of participating on this dig.
